Foreign travel
If you are planning a trip abroad, contact the surgery and one of our practice nurses will advise you on the protection you will need. Please complete a travel questionnaire form either online or printable version (see below for further details) at least EIGHT WEEKS before you travel, but if travelling within the next two weeks make an appointment now. Please read our top tips for travelling.

Some vaccines require special order and may take a few days to arrive, a Practice Nurse will advise.
Please be aware that there may be a charge for some vaccines which are payable in advance, please visit our fees page to see our list of charges. The preferred method of payment is by credit or debit card, a Practice Nurse will advise.
You can if you wish either complete and submit your travel questionnaire online; or print it out to complete and return to your usual surgery. Copies of the Travel Questionnaire are also available from reception. A practice nurse will respond within 5 working days.
