Our York patients can take advantage of Link Workers to support their health and wellbeing needs.
What is a Link Worker?
Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.
Link workers also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners.
Social prescribing works for a wide range of people, including people:
· with one or more long-term conditions
· who need support with their mental health
· who are lonely or isolated
· who have complex social needs which affect their wellbeing.

If it sounds like support that could be useful to you, there are a number of ways you can access further information.
· Speak to your GP
· Ask reception to refer you to your Link Worker
· Call the team on 01904 437911.
You can always have a brief conversation with a Link Worker to get a bit more information about how the process works, and what support they can offer you. If it doesn’t sound right for you at the current time, then you’re welcome to access support at another time.
